Multi-disease Treatment Plans

Coordinate patients ongoing chronic illnesses, medication regiment management, ordering specialized testing for diagnostic and follow up purposes, yearly wellness exams to customize medical care to create a plan for ongoing medical conditions. Ongoing blood work is available for monitoring and diagnosing chronic and acute care problems.

In-Patient Hospital and Out-Patient Care

We have providers from our practice that are on call 24-7 to meet and examine you in the hospital upon admission. We will manage and coordinating your care throughout your stay and continue with post discharge care at our office.

Nursing Home & Rehab Facility Care

Our doctors along with our advanced practice nurses continue care for our patients after their hospitalization with transition to skilled nursing and rehab facilities.
